Traveling to Kenya requires a bit more than just packing your bags. One of the essential documents you need to have is the Kenya Health Declaration Form. This form is a crucial part of the travel process, especially in the wake of the COVID-19 pandemic.


The Kenya Health Declaration Form is a document that foreign travelers to Kenya need to complete online. This requirement is under the Public Health Act CAP 242 of the laws of Kenya. The form is designed to monitor the health status of incoming travelers and mitigate the risk of spreading infectious diseases, particularly COVID-19.

Travelers are asked to confirm their COVID-19 Negative PCR test certificate or proof of vaccination against COVID-19 on the form. This is a crucial step in ensuring the safety of both the traveler and the Kenyan public.

The form is mandatory for all foreign travelers entering Kenya.

The form can be obtained when applying for a Kenyan eVisa online. Once the eVisa has been processed and approved, a QR code will be provided to your mail, which must be presented to port health officials upon arrival in Kenya.

To complete the online Health Surveillance form, travelers will be asked to provide the following mandatory personal information:

  • First name
  • Last name
  • Gender
  • Date of birth
  • Nationality
  • Country of origin
  • Date of arrival in Kenya
  • ID / Passport Number
  • Phone Number (with country code)

Optional personal information includes:

  • Middle name
  • Email address (to which the QR code will be sent)

Mandatory travel information required includes:

  • Airline
  • Flight Number
  • Seat Number

Optional Travel Information includes:

  • Destination City
  • Countries recently visited

Mandatory health questions include:

  • Do you have a fever of 37.5°C or higher?
  • Are you feeling feverish?
  • Do you have a cough?
  • Do you have a negative COVID-19 PCR certificate?
  • Do you have chills?
  • Do you have difficulty breathing?
